FHLMC Home Affordable Modification Program Documentation Now Available

Freddie Mac has issued the following update titled, " Home Affordable Modification Program Documentation Now Available".

As a follow-up to our March 11 Single-Family Seller/Servicer Guide Bulletin, today we are providing Servicers with some of the required documentation, and introducing two new forms, to help begin implementing the Home Affordable Modification program. As previously communicated, we will be providing you with all the documents required under the Home Affordable Modification program in the near future.

Program documents are available for you to download from a link on the Freddie Mac Home Affordable Modification program Web page. You must use your Mortgage Servicing Products ID to access the forms from a secure Web page. To obtain a Mortgage Servicing Products ID please visit our Servicing Applications Sign Up Form. The documents available for use later today include:

  • A Proactive Solicitation Letter, Form 1120, that may be used for borrowers who are 31 days or more delinquent.
  • A Documentation Request Letter, Form 1121, which may be used to collect additional required information from the borrower.
  • A Hardship Affidavit, Form 1122, to be signed by all borrowers.
  • A Workout Plan Cover Letter, Form 1123, for stated income.
  • A Workout Plan Cover Letter, Form 1124, for verified income.
  • NEW! A Counseling Referral Letter, Form 1119, that may be used to inform borrowers they are required to attend credit counseling.
  • NEW! A Special Inspector General Troubled Asset Relief Program (SIGTARP) Fraud Notice, Form 1125, that must be included with the Document Request Letter or the Workout Plan Cover Letter for stated income.

The following legal documents will be available to you in the near future. We will notify you when they become available. Upcoming documents include:

  • Home Affordable Modification Program Workout Plan, Uniform Instrument Form 3156.
  • Home Affordable Modification Program Modification Agreement, Uniform Instrument Form 3157.
  • An Agreement Cover Letter.

Servicers may download the Freddie Mac logo to co-brand envelopes and packages. Servicers may not use Freddie Mac's logo without also including its own logo and any other disclosures required by applicable federal, State, or local law (e.g., equal housing lender logo).
